Please arrive for your appointment 10 minutes before your actual schedule appointment time.

Bring all medications with you or a current list on your first visit.

Bring your insurance and pharmacy cards with you at every visit.

If for any reason you must cancel your appointment, we ask that you give us 24 hour notice to avoid a $75.00 no show/late cancellation fee.
